About The Position

The LTSS Service Coordinator - LPN Case Manager at Elevance Health is a field-based role responsible for coordinating care for members with chronic illnesses, disabilities, and co-morbidities. The position involves working under the supervision of an RN to assist in assessments, care plan development, and management of member needs, ensuring effective utilization of health benefits. Requirements

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN as allowed by state law.
• Nursing Diploma or Associate's degree in Nursing or a related field.
• Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities.
• Experience in a Service Coordinator or similar role.

Nice To Haves

• Bilingual in Spanish strongly preferred.
• Experience using Microsoft Office Suite including Outlook, Word, and Excel preferred.
• Previous case management experience preferred.

Responsibilities

• Assist the responsible RN with telephonic or face-to-face assessments for member needs.
• Identify members for high-risk complications and obtain clinical data as directed by the RN.
• Assist in identifying members that would benefit from alternative levels of care or waiver programs.
• Provide collected information to the RN for verification and interpretation.
• Participate in coordinating care for members with chronic illnesses and disabilities in conjunction with the RN and healthcare team.
