About the position

The Main Office Social Work Case Manager at AIDS Alabama is responsible for providing community outreach and client-centered case management services to individuals and families at risk of or experiencing homelessness. This role involves linking clients to community resources, ensuring eligibility for services, and building therapeutic relationships to help clients maintain permanent housing. The position requires a strong commitment to supporting diverse populations, including those living with HIV, and involves collaboration with landlords to secure affordable housing options.

Responsibilities

  • Provide case management services to a caseload of consumers seeking services, focusing on those at risk or experiencing homelessness.
  • Ensure documentation of consumer eligibility prior to service provision and maintain required documentation for continued eligibility.
  • Build therapeutic relationships with clients to assist in developing life skills necessary for maintaining permanent housing.
  • Provide outreach to potential landlords and manage relationships to ensure availability of safe, affordable housing.
  • Provide aftercare support to consumers in the program.
  • Complete and submit weekly billings and reports on time.
  • Adhere to quality practices and assist in recording and reporting outcome measurements for grants and agency reports.
  • Manage temporary client-centered special projects and assignments as requested.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in social work or related field from an accredited college or university.
  • Strong computer and case management skills.
  • Reliable transportation and valid Alabama driver's license with a good driving record.
  • Proof of COVID-19 vaccination.

Nice-to-haves

  • Master's degree in social work or related field from an accredited college or university.
  • Licensed to practice social work in the state of Alabama.
  • Two or more years working with persons experiencing homelessness.
  • Two or more years working in housing, preferably with Rapid Re-Housing experience.
  • Bilingual in English and another language.
  • Proven application of Trauma Informed Care, Motivational Interviewing, and/or Harm Reduction.
