Assertive Community Treatment (ACT) is a full spectrum, long term, community-based treatment model for adults with severe and persistent mental health needs that have not responded adequately to more traditional treatment services. ACT helps the person simultaneously in multiple life domains, such as medication management, mental health and substance use therapy, housing, employment, coordination of medical care and 24/7 on call support. ACT is an evidenced based treatment approach that involves a high level of coordinated team-based care, focused on supporting clients to obtain whole health wellness and independence. The ACT Team Manager is the clinical and administrative supervisor of the team. They provide a combination of leadership, clinical and case management supervision, program evaluation and CQI services to establish, support and continuously improve and maintain a high performing, high fidelity ACT team. The ACT Team Manager is primarily responsible for bringing the team services together, including psychiatry, med management, nursing, mental health, substance use, vocational, peer services, case management, and crisis response. The ACT Team Manager also holds primary responsibility for program sustainability through effective service utilization and ensuring that all services are delivered within the designated operating budget.
