About the position

The Manager, Clinical Operations at ACT One Health is responsible for leading multidisciplinary teams to provide community care for high-risk patients. This role involves day-to-day management of Care Teams, ensuring adherence to clinical protocols, and collaborating with internal and external partners to enhance patient outcomes and experiences. The position requires flexibility and a commitment to improving healthcare delivery in a dynamic environment.

Responsibilities

• Establish appropriate clinical processes for patient onboarding and assessments.
• Measure and manage key performance indicators for the Care Team.
• Lead day-to-day management of the Care Team, addressing priorities and issues.
• Monitor caseloads and adjust assignments as necessary.
• Lead daily clinical huddles and weekly interdisciplinary care team rounds.
• Cultivate a strong internal culture focused on coaching and collaboration.
• Participate in the hiring process and lead clinical training.
• Monitor and manage key reporting metrics, meeting with leadership weekly.
• Collaborate with internal business partners to ensure compliance with service agreements.
• Support client relationships through meetings and reporting outcomes.

Requirements

• Management experience in a clinical operations environment.
• Minimum of three years of clinical staff management experience.
• Experience managing multidisciplinary care teams.
• Strong analytical skills for data-driven decision making.
• Excellent written, computer, and oral communication skills.
• Ability to prioritize and handle multiple tasks in a fast-paced environment.
