About the position

The MDS Coordinator LPN is responsible for overseeing patient assessments, coordinating care plans, and ensuring compliance with regulatory requirements in a long-term care facility. This role involves administrative duties, drug administration, personnel management, and direct nursing care, while also participating in quality assurance programs and facility surveys. The MDS Coordinator plays a crucial role in maintaining the standards of care and ensuring that nursing personnel adhere to established policies and procedures.

Responsibilities

  • Administer patient assessments and oversee the assessment process.
  • Coordinate care plans according to regulatory requirements.
  • Create schedules for Medicare and Medicaid assessments.
  • Direct day-to-day functions of nursing assistants in compliance with regulations.
  • Ensure compliance of nursing personnel with facility policies and procedures.
  • Participate in the development and implementation of the quality assurance program.
  • Assist in planning nursing services for resident discharge plans.
  • Complete required recordkeeping forms and charts upon resident admission, transfer, and discharge.
  • Prepare and administer medications as ordered by physicians.
  • Participate in employee performance evaluations and staffing requirements.

Requirements

  • Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved LPN/LVN program.
  • Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
  • Must be able to read, write, speak, and understand the English language.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents.

Nice-to-haves

  • Knowledge and experience with PCC preferred.
  • Current CPR certification.
