About the position

The Care Team Associate I serves as a vital member of the Care Team, providing essential non-clinical services and administrative support focused on population health. This position operates under the supervision of the Practice Administrator and collaborates closely with the Nurse Manager. A fundamental understanding of basic medical terminology is required, along with a commitment to delivering exceptional customer service. The Care Team Associate I plays a crucial role in ensuring that patients receive the highest quality of care by supporting the care team in various capacities. In this role, the Care Team Associate I is responsible for warmly greeting patients and guests, engaging in friendly conversation, and effectively communicating wait times. They monitor patient flow in the waiting room, proactively resolving any issues related to registration, check-in, and check-out processes. The associate assists patients in addressing service or clinical care concerns, ensuring a smooth experience from arrival to departure. The position requires answering telephones professionally, scheduling appointments accurately, verifying insurance coverage, and resolving patient concerns. The Care Team Associate I is responsible for checking in patients, processing necessary forms, and maintaining the integrity of patient data within electronic systems. They also handle patient check-out, ensuring all required documentation is complete and soliciting feedback on the service provided. Additionally, the associate coordinates referrals to specialists, manages follow-up communications, and assists with health campaigns and initiatives. They maintain effective communication with patients, families, and healthcare team members, demonstrating flexibility and initiative in their work. Proficiency in electronic health record systems is expected within three months of training, along with adherence to professional standards and confidentiality protocols. The Care Team Associate I contributes to a supportive work environment and performs other job-related duties as required.

Responsibilities

  • Warmly greets and engages in friendly conversation with patients and guests.
  • Communicates approximate wait times and maintains timely communication with patients and guests.
  • Monitors patient flow in the waiting room and assesses needs as they wait.
  • Proactively resolves patient flow issues related to registration, check-in, and check-out processes.
  • Assists patients in resolving service or clinical care issues as needed.
  • Answers telephones professionally, utilizing excellent customer service skills.
  • Schedules patient appointments accurately in the computer system and verifies insurance coverage.
  • Checks-in patients and prepares and processes appropriate forms.
  • Serves as a reference source regarding hours of operation, directions, and general policies.
  • Performs complete registration of patient's demographic and financial information using electronic systems.
  • Checks-out patients at the end of the visit, verifying required documentation before they leave.
  • Thanks patients as they leave and solicits feedback about their experience.
  • Takes complete and accurate messages from individuals contacting the medical facility by telephone.
  • Delivers messages to the intended recipient in a timely manner and escalates calls as necessary.
  • Collects cash from patients for charges and co-payments and provides receipts.
  • Completes necessary documents for all payments and performs end-of-day reconciliations.
  • Coordinates internal and external referrals to specialists, scheduling appointments and providing reminders.
  • Acts as a liaison between the practice and external specialists for outpatient procedures.
  • Assists care teams with health campaigns and initiatives, providing information to patients.
  • Maintains effective communication with patients, families, and healthcare team members.
  • Attends meetings as directed and demonstrates flexibility in work assignments.
  • Demonstrates proficiency with computer systems within three months of training.
  • Arrives on time for assigned shifts and maintains punctuality and attendance records.
  • Respects patient confidentiality and demonstrates good judgment and discretion.

Requirements

  • High School diploma or GED required.
  • 3-5 years' experience in customer service/care coordination in an ambulatory care setting.
  • Experience and ability to navigate an EMR.

Nice-to-haves

  • Associate degree preferred.
