About the position

The Memory Care Leader at The Oaks at Jamestown is responsible for overseeing the daily operations of the Legacy Lane programs, focusing on providing exceptional care and education for residents with dementia. This role involves managing staff, ensuring compliance with care standards, and fostering relationships with residents and their families to enhance the quality of life for those in memory care.

Responsibilities

  • Coordinates daily operations of the Legacy Lane programs with the Executive Director and leadership team.
  • Manages the Best Friends Forever (BFF) Program, including team education.
  • Ensures Daily Rhythms activity programming is implemented according to company standards.
  • Maintains the Department Leader Rounding binder and collaborates with the Executive Director for follow-up.
  • Assists in maintaining appropriate resident population as per memory care criteria.
  • Participates in memory care Resident First Meetings and guides family members through care options.
  • Develops relationships with resident family members to ensure excellent customer service.
  • Provides Family Orientation Guide to new family members.
  • Maintains communication with staffing coordinator for consistent dementia staffing.
  • Interviews potential team members to assess their suitability for working with dementia residents.
  • Maintains a dementia-safe environment through regular rounds and management of stimuli.
  • Oversees meal service in accordance with Family Style Dining Standards.
  • Educates campus staff and the community on dementia-related topics as a Dementia Champion.
  • Partners with marketing to ensure First Impression Standards are met.

Requirements

  • Knowledge of dementia care principles and practices.
  • Understanding of Medicare and Medicaid guidelines.
  • Ability to develop a servant leadership culture.
  • Strong organizational skills with attention to detail.
  • Effective communication skills, both oral and written.
  • Experience in assessing resident psychosocial needs and developing care plans.
  • Ability to maintain confidentiality and professional discretion.

Nice-to-haves

  • Previous experience in dementia care or activities.
  • C.N.A or QMA/KMA/Med Tech certification.
  • LPN/RN license in the state of employment.
