About the position

The MSW Rehab Social Worker at Hope Network plays a crucial role in providing clinical services to individuals and families facing challenges related to rehabilitation. This position involves conducting evaluations, treatment planning, and discharge planning, while also leading interdisciplinary teams to ensure effective support for clients. The social worker will work closely with families to facilitate smooth transitions and connect them with necessary community resources, ultimately aiming to enhance the quality of life for those served.

Responsibilities

  • Provide clinical services including evaluation, diagnosis, psychosocial assessments, treatment planning, family assessment, risk assessments, and discharge planning.
  • Lead the interdisciplinary team in assessing the ability of individuals and families to cope with challenges and support long-term goals.
  • Provide families with information and updates on progress toward goals.
  • Identify and document the patient's pre-injury lifestyle, abilities, and interests to maximize meaningful outcomes.
  • Ensure smooth transitions for individuals discharging from the program to home or other care levels, including housing and community resource connections.
  • Assist families and guardians with SSDI/Medicaid applications and medical benefits.
  • Serve as a primary therapist when designated, providing individual or group counseling and psycho-emotional services.
  • Provide consultation and training to staff regarding interpersonal, social, and behavioral functioning of individuals and families.
  • Participate in developing psychosocial curriculum and family services.
  • Document activities for billing and ensure timely completion of clinical reports.

Requirements

  • Master's degree in Social Work (M.S.W.) from an accredited college or university required.
  • Licensed as a Social Worker by the State of Michigan (Clinical designation).
  • At least two years of experience in a health/human service field.

Nice-to-haves

  • Experience in substance abuse intervention and education.
  • Familiarity with crisis management and psychosocial consultation.
