About The Position

The Nurse Liaison at Hospice of Davidson County is a vital role focused on educating and promoting hospice services within the medical community. This position is responsible for building and maintaining relationships with hospitals, physician groups, and other healthcare facilities to enhance awareness and referrals for hospice services. The Nurse Liaison will facilitate a smooth referral and admission process for patients and ensure continuity of care for those admitted to hospice services.

Requirements

  • Associate degree or nursing school diploma required.
  • Bachelor of Science in Nursing preferred.
  • Certification in Hospice and Palliative Care preferred.
  • Licensed as a registered nurse in North Carolina.
  • Two years current nursing experience required.
  • Maintain current unrestricted North Carolina driver's license.
  • Ability to work as a hospice team member in a flexible work environment.
  • Must be physically capable of providing patient care in various institutional settings.
  • Must be certified to perform CPR.
  • Must meet immunization requirements of North Carolina and Davidson County.

Responsibilities

  • Educate and promote hospice services to the medical community.
  • Facilitate a smooth referral and admission process to hospice from area facilities.
  • Ensure continuity of the hospice plan of care for patients admitted to area hospitals.
  • Enhance relationships with key referral sources by providing education and consistent contact.
  • Act as a liaison between hospice and area facilities to enhance physician satisfaction and ensure optimal patient care.
  • Meet with referred patients and families to explain hospice services and complete pre-assessments.
  • Complete initial assessments and collaborate with the interdisciplinary team to establish the plan of care.
  • Contact the patient's attending physician and Hospice Medical Director regarding certifications and orders for the plan of care.
  • Teach and counsel patients/families about the disease process and medication management.
  • Complete daily nursing visits on hospice patients while under general inpatient care in area hospitals.
  • Communicate daily with hospital nursing staff, case management staff, and physicians to facilitate discharge plans.
  • Maintain accurate records of patient/family care services per hospice policy.
  • Participate in staff meetings, interdisciplinary team conferences, and educational programs.
  • Participate in the Quality Assessment/Performance Improvement process and the ACHC accreditation process.
