About the position

ServiceNet is seeking dedicated and compassionate Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to join our team in West Springfield, MA. This full-time position involves working with individuals who are developmentally disabled or have suffered brain injuries, providing essential nursing care within our residential programs. The Nursing Team plays a crucial role in advocating for the health and well-being of our clients, ensuring that all medical needs are met through appropriate nursing interventions. The position is primarily Monday to Friday, from 9 AM to 5 PM, allowing for a structured work schedule while making a significant impact in the lives of our clients. As a Nurse Monitor, you will be responsible for overseeing the medical care of program participants, which includes conducting quarterly audits and monitoring significant client medical appointments. You will coordinate healthcare services, ensuring that all medical issues are communicated effectively to physicians, nurse practitioners, and other healthcare providers. Documentation is a key aspect of this role, as you will be required to maintain accurate records of nursing interventions, changes in client conditions, and medication orders in compliance with MAP standards. Additionally, you will assist in training staff on personal care and the use of supportive devices, contributing to a collaborative and supportive environment for both clients and staff. Responsibilities

  • Oversight of Residential Programs within Western MA.
  • Conduct quarterly audits of client care.
  • Monitor and coordinate healthcare services for participants.
  • Institute appropriate follow-up and communicate all medical issues to relevant healthcare providers.
  • Document all nursing interventions and changes affecting the individual in appropriate areas.
  • Maintain current medication and treatment orders according to MAP standards.
  • Assist in training staff in personal care, positioning, feeding, and use of supportive devices.
  • Complete additional assignments as directed by management.

Requirements

  • High School Diploma / GED required.
  • Valid MA Nursing License (LPN or RN).
  • Valid driver's license and acceptable driving record.
  • Physical ability to perform the requirements of the position.
  • Good communication and computer skills.
