Responsibilities

• Conduct in-home health risk assessments for members in your geographic area.
• Evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams.
• Use evidence-based practice screening, including point-of-care testing as needed.
• Identify diagnoses for care management by Porter Care Guides.
• Actively manage medical treatment based on assessment findings.
• Formulate lists of current and past medical conditions using clinical knowledge and judgment.

Requirements

• Valid unencumbered Michigan NP license.
• 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred).
• A strong sense of empathy, compassion, and friendliness.
• Exceptional communication skills, both written and oral.
