About The Position

The Nurse Practitioner position at Matrix Medical Network is a PRN/Part Time role focused on conducting home risk assessments for at-risk health plan members. Requirements

• Master's Degree required or equivalent experience and satisfactory completion of NP licensure.
• Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it.
• Board certified by AANP, ANCC, or AACCN in a Matrix approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and documentation according to Matrix guidelines.
• Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
• Collaborate with Primary Care Physicians on patient education and follow-up care.
• Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
