Responsibilities

• Manage clinical care for residents at assigned skilled nursing facilities (SNFs).
• Round daily and prescribe medications and treatments for patients.
• Provide comprehensive primary care services to patients in SNFs.
• Document all patient care encounters in the electronic health record (EHR) system.
• Be available for consultations with facility nursing staff via phone or secure messaging.
• Ensure timely attendance for scheduled shifts.
