The Nurse Practitioner (NP) at ChenMed plays a crucial role in the clinical operations team, providing direct patient care in dedicated medical centers. This position is designed for individuals who are innovative, entrepreneurial-minded, and possess a strong work ethic. The NP is responsible for conducting geriatric assessments, taking medical histories, performing physical exams, diagnosing and treating patients, and developing comprehensive nursing care plans. Health education, physician referrals, and case management referrals are also key components of the role, along with thorough documentation that adheres to ChenMed's standards for quality, service, productivity, and teamwork. In addition to direct patient care, the Nurse Practitioner participates in clinical rounds and conferences, ensuring that all documentation is completed accurately and in a timely manner. The NP must demonstrate the ability to work independently while also collaborating with other healthcare professionals. This includes consulting with managers, physicians, and medical directors to ensure compliance with established guidelines. The role may also involve participation in risk and quality management programs, clinical meetings, and other relevant gatherings. The NP is expected to independently assess both acute and non-acute clinical problems, document physical assessments and patient histories, analyze trends in patient conditions, and develop and implement patient management plans based on the data collected. This includes writing admission, transfer, and discharge orders, ordering and interpreting laboratory and diagnostic studies, and prescribing appropriate medications and treatments. The NP serves as a patient advocate, ensuring that care is tailored to the individual needs of each patient, taking into account their cultural background and support systems. The position requires strong critical thinking and problem-solving skills, excellent communication and interpersonal abilities, and the capacity to manage time effectively under pressure. The NP must be caring, empathetic, and detail-oriented, with basic computer skills and familiarity with Microsoft Office Suite. This role may require local travel of 50-75%.
