About The Position

The Nurse Clinician at Bridgeway is responsible for providing outpatient nursing services to mentally ill consumers, utilizing various therapeutic modalities. This role involves case management, medication monitoring, and ensuring comprehensive healthcare services while collaborating with other departments to resolve issues effectively.

Requirements

• Associate's Degree in an accredited nursing program.
• Valid nursing license in Illinois.
• Prior experience with elderly clients is preferred.
• One year of mental health experience is preferred.
• Reliable transportation and valid driver's license that meets organization's insurability requirements.

• Knowledge of medication management and mental health interventions.

Responsibilities

• Provide informed consent regarding medication purpose, side effects, and adverse reactions to Medication Clinic consumers, renewing consent annually.
• Assist in the coordination and maintenance of a tardive dyskinesia monitoring system for consumers in accordance with Rule 132 and CARF standards.
• Provide case management services, medication monitoring, and training to mentally ill consumers, ensuring adherence to treatment plans and proper documentation.
• Assess consumers' knowledge of medication and develop goals for understanding their mental illness and medication management.
• Monitor consumers' physical and mental conditions, providing necessary interventions to promote wellness and prevent decompensation.
• Assist in the operation of the medication clinic, including client assessment, administration of long-acting injectable neuroleptics, and maintaining clinic records.
• Implement the Clozaril management system in conjunction with psychiatric services following established protocols.
