About the position

The Occupational Therapist - Home Health position at PeaceHealth in Florence, Oregon, is responsible for evaluating patients, determining treatment plans, and participating in patient care within the scope of Occupational Therapy. This role involves collaboration with a patient care team to achieve optimal outcomes and may include oversight of therapy assistants and students.

Responsibilities

  • Evaluates assigned patients and establishes a treatment plan and goals using data from various sources.
  • Completes patient treatment according to the plan of care and modifies it as needed.
  • Accurately documents patient care in compliance with regulatory and state licensure requirements.
  • Collaborates with healthcare professionals to ensure continuous patient care and acts as a liaison for problem-solving.
  • Mentors and provides oversight to new caregivers and therapy assistants, and may provide clinical instruction to students.
  • Participates in the development of new services and quality improvement activities.
  • Maintains appropriate clinical records and participates in staff meetings and educational in-services.

Requirements

  • Bachelor's, Master's, or Doctorate Degree in Occupational Therapy accredited by the Accreditation Council for Occupational Therapy Education.
  • Occupational Therapist License in the State of Oregon.
  • Driver's License if working in Oregon.
  • Basic Life Support certification required upon hire.

Nice-to-haves

  • One year of experience as an Occupational Therapist.
  • Home Health experience.
