About the position

You will function as a patient advocate, serving as the initial contact for patient-related issues and clarifying and implementing physician orders. Your responsibilities will include conducting patient assessments, collaborating with multidisciplinary teams, and ensuring that the physical, cultural, and psychosocial needs of patients and their families are met. You will also be responsible for maintaining patient confidentiality and performing required documentation. As a contributing member of our healthcare team, you will assist with the daily oversight of other clinical staff, providing mentorship and support to new staff members. Responsibilities

  • Utilizes current nursing practices to provide total patient care while focusing on the needs of the patient.
  • Functions in the role of patient advocate by being the initial contact for patient-related issues.
  • Clarifies and implements physician orders by administering medications and performing/ordering treatments and testing as prescribed by the physician.
  • Reviews and evaluates the plan of care, obtaining information from the patient and/or family, and adds to the plan of care as appropriate.
  • Collaborates with the multidisciplinary team, the patient, and family to promote efficient patient care.
  • Provides for the physical, cultural, and psychosocial needs of the patient and family.
  • Integrates evidence-based practice and professional standards whenever possible.
  • Serves as a champion for patient safety by continually observing and monitoring the clinical environment.
  • Performs required documentation and communicates effectively to meet patient and/or family needs and regulatory requirements.
  • Protects confidentiality of patient's Personal Health Information (PHI).
  • Assists with the daily oversight of other clinical staff when directed by the manager.
  • Provides feedback to the manager regarding work-related skills of non-licensed personnel.
  • Serves as a mentor and preceptor for new staff and as a resource to non-licensed personnel.
  • Remains flexible with work assignments throughout the clinical site to assure appropriate coverage.

Requirements

  • Current and unrestricted LPN license in Illinois
  • At least 1 year of hospital experience
  • Experience with clients on ventilators and maintaining the settings
  • Experience with tracheotomy care and suctioning
  • Ability to work overnights
  • Ability to report changes/abnormalities to family & physician
  • Ability to crush prescribed medications
  • Ability to assist with nebulizer treatments
  • Ability to perform dressing changes
  • Ability to work 12-hour shifts including weekends: 8:00 AM - 8:00 PM or 8:00 PM - 8:00 AM
  • Proof of eligibility to work in the US
  • Reliable car and auto insurance
  • Clean background and criminal record
  • Recent Physical, TB/Chest X-ray on record
