About the position

The IDT Moderator at San Ysidro Health plays a crucial role in ensuring that the Interdisciplinary Team (IDT) operates effectively, making decisions about medical, social, and supportive services without external influences. This position involves collaboration with various team members, oversight of performance measures, and ensuring compliance with PACE regulations. The IDT Moderator is responsible for fostering teamwork, coordinating care plans, and enhancing the quality of services provided to participants.

Responsibilities

• Meet regularly with Site Dyad Leadership and Technician Managers to review goals, trends, and performance measures for each team member.
• Promote the mission and vision of leadership positively and productively.
• Coordinate the hiring, training, and evaluation of IDT members with the Technician Manager.
• Provide input in annual performance reviews related to IDT and team performance.
• Ensure team members understand their roles and the importance of teamwork.
• Emphasize collaboration in all interdisciplinary and cross-team interactions.
• Coordinate orientation and in-service programs for staff to meet regulatory requirements.
• Ensure quality services are provided within expected standards and financial targets.
• Facilitate activities related to new participants, including orientation.
• Coordinate all IDT Team Meetings, including care planning and implementation.
• Monitor and track grievances and complaints from participants to identify trends.
• Assist leadership in service recovery efforts.

Requirements

• Bachelor's degree in a healthcare-related field or equivalent work experience.
• Minimum 1 year of experience working with an Interdisciplinary Team.
• Minimum of 2 years of experience working with the frail elderly population.
• CA Driver's license with appropriate insurance coverage.
• Good written and oral communication skills.
• Knowledge of medical terminology and Microsoft Windows.
