About the position

The PACE Medical Director is a progressive physician leadership role responsible for achieving the best clinical outcomes possible for all PACE participants. The Medical Director is responsible for the delivery of participant care, clinical outcomes, and the implementation as well as oversight of the quality assessment and performance improvement program. The PACE Medical Director is a champion of the PACE model of care as well as Age-Friendly healthcare across the continuum. This position requires excellent clinical, communication, and leadership skills spanning all spans of medicine, population health, social determinants, payer models, and compliance.

Responsibilities

  • Responsible for delivery of participant care, clinical outcomes, and oversight of quality for PacificSource PACE Organization.
  • Act as the site clinical lead, with assistance from an Associate Medical Director, as required, for PACE locations.
  • Understand and be accountable for the PACE financial model including the medical economics of capitated care.
  • Direct oversight of site physicians and Advanced Practice Practitioners (APP's) at PACE Center(s).
  • Participate in the hiring, termination, and management of PACE clinical team members including physicians and APP's.
  • Work closely with and provide leadership support to the PACE Quality Manager, including with development of the PACE Quality Plan.
  • Work collaboratively with PacificSource business units including quality, compliance, Human Resources, and others to ensure high performance in accordance with PacificSource standards.
  • Active participation in all CMS and State Audits including quarterly calls.
  • Development of care pathways and clinical practice guidelines to effectively manage chronic health conditions and avoid preventable hospitalizations, emergency department usage, and premature skilled nursing facility placement.
  • Direct patient care, including basic office-based, ambulatory procedures, telehealth, physical examinations, evaluation, and management based upon the needs of the PACE program and as directed by the PACE CMO.
  • Develop and provide Age-Friendly and goal-concordant participant care.
  • Direct oversight of students and residents of Medical Schools, residencies, Nurse Practitioner, and Physician Assistant programs.
  • Fulfill all administrative duties on a timely basis, including record keeping in participant charts, annual performance evaluations, corrective action plans, and strategic planning.
  • Work in close collaboration with the PACE Nursing leadership, Executive Director, and Clinic Manager on the delivery of care within the clinic and PACE participants' homes.
  • Conduct timely peer-to-peer discussions to clarify clinical information and to explain review outcome decisions across the continuum of care.
  • Foster a safe and supportive environment conducive to the professional development of healthcare professionals in accordance with principles of High Reliability.
  • Serve on various internal and external committees as required or designated.
  • Actively participate and lead the PACE IDT to develop and implement a comprehensive, plan of care for each participant upon enrollment and throughout the participant's enrollment as specified by PACE regulations.
  • Advance Care Planning (ACP) and palliative approaches to participants in the context of goal-concordant care.
  • Clear and accurate documentation within the Electronic Health Record (EHR) including all communication and decision-making as specified by PACE regulations including oversight of physicians and APP documentation in the EHR with auditing as indicated.
  • Advise on care management and examine patients with other PACE physicians and APP's as indicated to mentor, support, and lead.
  • Collaborate with specialists including dentistry, podiatry, chiropractic, therapies (speech, physical, occupational), and medical subspecialties.
  • Coordinate care across all settings including hospitals, skilled nursing facilities, and assisted living facilities and closely collaborate with other attending physicians.
  • Visit participants in their home or care setting as indicated.
  • Prescribe pharmaceuticals and treatment regimens as appropriate to assessed medical conditions.
  • Refer to and coordinate with specialists as appropriate.
  • Provides all services in a manner that is consistent with PacificSource core values and State and Federal regulatory requirements.
  • Participate in team meetings, caregiver groups, and special events such as indicated.
  • Provide on-call coverage for direct patient care after hours and weekends as indicated.
  • Meet department and company performance and attendance expectations.
  • Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
  • Perform other duties as assigned.

Requirements

  • Minimum one-year experience working with the elderly or frail population required.
  • Minimum five years of clinical experience as an attending physician preferred.
  • Experience in a value-based healthcare organization, payer model, or closely related field preferred.
  • Prior experience in PACE, Skilled Nursing, or a managed Medicaid setting is preferred.
  • At least two years of physician leadership experience is preferred.
  • Doctor of Medicine (MD or DO) with an unrestricted license to practice medicine by the Oregon State Board of Medicine required.
  • Required board certification recognized by American Board of Medical Specialties (ABMS).
  • Current certification in Basic Life Support or Advanced Cardiac Life Support required.
  • Current and valid Drug Enforcement Agency Certificate required.
  • Valid Oregon Driver's License Required.
  • Certified Medical Director by the American Board of Post-Acute and Long-Term Care Medicine preferred.
