About the position

The Pediatric Nurse Practitioner/Physician Assistant at La Clinica de La Raza is responsible for delivering patient-centered primary care in an outpatient setting. This role involves providing comprehensive medical care to a diverse and medically complex patient population, with a focus on achieving optimal health outcomes. The position emphasizes collaboration within a care team and offers opportunities for mentorship and professional development.

Responsibilities

  • Provide medical triage of patients, medical care management, and patient education and counseling.
  • Make referrals to specialty providers and follow-up to ensure that patients' bio-psychosocial needs are addressed.
  • Deliver evidenced based care that supports improved health outcomes and exceptional patient experience.
  • Perform routine, acute, urgent, preventive, curative, and rehabilitative medical care to patients.
  • Serve as a member of the cross-functional team of providers engaged in routine and preventive primary care.
  • Prescribe medications, treatments or therapies and order medical diagnostic or clinical tests.
  • Deliver well and chronic disease care with cultural humility to a diverse patient population.
  • Develop a positive rapport with patients and families to foster the provider/patient relationship.
  • Conduct in-home assessment of patients as appropriate.
  • Consult with physicians on the management, assessment and treatment of specific cases as needed.

Requirements

  • Current BLS (Basic Life Support) Certificate.
  • Board certification required; candidates may be eligible for hire without it but must obtain it within 6 months.
  • Valid California license and DEA certification required.
  • For Nurse Practitioners: Current license from the California Board of Registered Nursing and national certification from a recognized body.
  • For Physician Assistants: Current license from the California Physician Assistant Committee and graduation from an approved program.

Nice-to-haves

  • Proficiency in English and Spanish preferred but not required.
  • Experience with Electronic Health Record programs, such as NextGen or Epic.
  • Cultural competence working in diverse/low income communities.
