About the position

The Pharmacist plays a crucial role in ensuring excellent patient outcomes and satisfaction within the hospital pharmacy. This position is responsible for overseeing daily operations to meet customer and patient expectations effectively. The pharmacist will contribute to the development of pharmacy-specific policies and procedures, ensuring compliance with all applicable Joint Commission and other auditing agency regulations. This role also involves active participation in an interdisciplinary team, providing essential information to the team and utilizing feedback to ensure positive patient outcomes. In this acute rehabilitation hospital setting, the pharmacist will encounter a diverse patient population with various diagnoses, including stroke, spinal cord injury, brain injury, neurological disorders, rheumatoid arthritis, osteoarthritis, amputation, joint replacement, fractures, multiple trauma, cardiac conditions, and pulmonary conditions. Notably, 80% of patients are discharged directly home. The pharmacist will utilize evidence-based practice guidelines to treat patients and will engage in continuous improvement of treatment interventions and programs by analyzing clinical outcomes. Participation in a national outcomes database will provide benchmarks and sophisticated outcome reports to help enhance patient outcomes. To support professional development, the pharmacist will be paired with mentors and will have opportunities to attend both in-house and external educational programs. The treatment team consists of full-time therapists, nurses, social workers, pharmacists, and medical staff, providing a collaborative environment for learning and growth beyond individual disciplines.

Responsibilities

  • Ensure daily operations within the hospital pharmacy meet customer and patient expectations.
  • Develop and contribute to pharmacy-specific policies and procedures in compliance with regulations.
  • Participate in the interdisciplinary team to provide and utilize information for positive patient outcomes.
  • Accurately interpret medication orders and compound and dispense medication preparations according to legal and professional standards.

Requirements

  • Licensed in good standing by the appropriate state pharmacy board.
  • Successful completion of a degree from an accredited pharmacy program or equivalent.
  • Minimum of 1 year of supervisory experience.
  • Good visual acuity and ability to communicate effectively.
