As a Staff Physical Therapist, you will examine patients using fundamental clinical knowledge, implement treatment plans, and execute the plan of care on a daily basis. In this role, you will be expected to collect and record patient care data, prepare for evaluations, select and administer tests, establish treatment plans, and collaborate with an interdisciplinary team. You will also be responsible for providing education and training to patients and their support systems, ensuring that interventions are safe, effective, and compliant with JAG's standards. Responsibilities

  • Collect and record patient care data following guidelines of patient care service, regulatory agencies, and third-party payers.
  • Prepare for evaluations by obtaining patient history and exhibiting knowledge of patient diagnoses.
  • Select and administer tests and measures, synthesizing evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
  • Establish treatment plans based on evaluation results, goals, and best practices.
  • Collaborate with interdisciplinary teams, patients, and their support systems.
  • Assess and re-assess the need for modifications to the plan of care and goals.
  • Identify the need for consultations and refer patients to other healthcare professionals or services.
  • Initiate and complete discharge plans, providing education/training to patients and their support systems.
  • Provide functional training, including ADLs/IADLs, therapeutic activities, and rehabilitation equipment prescription.
  • Oversee support staff including PTA, ATC, and PT aides.
  • Attend annual JAG Billing and Coding seminars and promote JAG in the community through lectures/presentations.

Requirements

  • Bachelor's or Master's Degree required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state regulated CEU requirement in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with EMR software is preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and individually with minimal supervision.
  • Maintain a positive, professional demeanor at all times.
