As a Staff Physical Therapist, you will examine patients using fundamental clinical knowledge, implement treatment plans, and execute the plan of care on a daily basis. Responsibilities

  • Collect and record patient care data following the format and guidelines of the patient care service, regulatory agencies, and third-party payers.
  • Prepare for evaluations in accordance with department policies and standards of practice, including obtaining patient history and knowledge of patient diagnoses.
  • Select and administer tests and measures, synthesizing evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
  • Establish treatment plans based on evaluation results, goals, and best practices.
  • Collaborate with interdisciplinary teams, patients, and their support systems.
  • Assess and re-assess the need for modifications to the plan of care and goals.
  • Identify the need for consultations and refer patients to other healthcare professionals or services.
  • Initiate and complete discharge plans, providing education and training to patients and their support systems.
  • Provide functional training, including ADLs/IADLs, therapeutic activities, rehabilitation equipment prescription, and community reintegration.
  • Responsible for treatment, plan of care, and exercise prescription for patient caseload.
  • Provide safe and effective interventions in compliance with JAG's standards.
  • Oversee support staff including PTA, ATC, and PT aides.
  • Attend annual JAG Billing and Coding seminar and promote JAG in the community through lectures and presentations.

Requirements

  • Bachelor's or Master's Degree required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state regulated CEU requirement in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience/knowledge with EMR software is preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and individually with minimal supervision.
  • Maintain a positive, professional demeanor at all times.

Nice-to-haves

  • Experience with EMR software is preferred.
