About the position

As a Home Health Physical Therapist at Ascension at Home Together with Compassus, you will work one-on-one with patients in their homes, providing essential care that helps them gain strength and independence. Your role is crucial in enabling patients to participate in daily activities, enhancing their quality of life. Responsibilities

• Provide one-on-one physical therapy to patients in their homes.
• Assess patients' needs and develop personalized treatment plans.
• Monitor patients' progress and adjust treatment plans as necessary.
• Educate patients and their families on exercises and techniques to improve mobility and strength.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

• Bachelor of Science in Physical Therapy.
• Current State License as a Physical Therapist.
• Valid Driver's License.
• Current CPR certification, negative TB screen, and Hepatitis consent/declination.
• Two years' experience as a Physical Therapist in an acute care or rehabilitation setting.
