About the position

The Physical Therapist (PT) at Diamond Ridge Health and Rehabilitation Center is responsible for assessing, treating, and educating patients with physical injuries or illnesses. This role involves providing physical therapy services as prescribed by referring physicians and collaborating with support staff to implement treatment programs. The PT aims to maximize patient independence and health through skilled interventions and assessments.

Responsibilities

  • Assess and treat patients as per physician orders.
  • Educate patients and their families about treatment plans and physical therapy processes.
  • Delegate appropriate treatment tasks to physical therapist assistants and support staff.
  • Conduct observational, verbal, or manual assessments of patients' musculoskeletal or neuromuscular systems.
  • Recommend and provide treatments based on assessment results, including range of motion and functional abilities.
  • Utilize purposeful activities to enhance patient independence and health maintenance.

Requirements

  • Must be licensed (or eligible for licensure) to practice as a physical therapist.
  • Prior experience in Skilled Nursing is preferred.
  • Membership in the state and national American Physical Therapy Association is preferred.
  • Professional attitude and appearance are required.
  • Proficient administrative and public relations skills are necessary.
  • Thorough knowledge of physical therapy principles, methods, materials, and equipment is essential.
  • Understanding of patient safety hazards and necessary precautions is required.
  • Demonstrate emotional stability to manage patient and employee demands.
  • Maintain confidentiality of patient and employee information.
  • Knowledge of Medicare and third-party reimbursement is preferred.
  • Successful completion of an accredited physical therapy educational program is required.

Nice-to-haves

  • Experience in a skilled nursing facility is preferred.
  • Membership in the American Physical Therapy Association is preferred.
  • Knowledge of Medicare and third-party reimbursement processes is preferred.
