About the position

PeaceHealth Rehab and Wellness Center in Florence, Oregon is seeking a Physical Therapist for a Full Time, 1.00 FTE, Day position. This role involves a combination of inpatient and outpatient care, working closely with a Physical Therapist Assistant (PTA) to cover acute care, swing bed, and outpatient services. The position is part of a supportive and collaborative team of therapists dedicated to providing best practice patient care.

Responsibilities

• Evaluates assigned patients and establishes a treatment plan and goals using data from various sources.
• Provides patient treatment according to the plan of care and modifies it to meet patient needs.
• Completes documentation accurately and timely according to regulatory and state licensure requirements.
• Collaborates with other healthcare professionals to ensure smooth continuous care of patients.
• Mentors and provides oversight to new caregivers, physical therapist assistants, and/or therapy aides.
• Recommends and participates in the development of new services and programs, quality improvement, and monitoring activities.
• Participates in staff meetings, patient care meetings, educational in-services, and other professional activities.

Requirements

• Bachelor's, Master's or Doctorate Degree in Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field.
• Physical Therapist License in the State of Oregon.
• Basic Life Support certification required upon hire.
