About the position

The Physical Therapist at JAG Physical Therapy is responsible for assessing patients' strengths and weaknesses, creating effective treatment plans, and providing high-quality care in a supportive environment. The role emphasizes patient confidentiality, positive encouragement, and collaboration with an interdisciplinary team to maximize patient recovery and functional independence.

Responsibilities

  • Collect and record patient care data following guidelines and regulations.
  • Prepare for evaluations by obtaining patient history and understanding diagnoses.
  • Select and administer tests and measures to evaluate patients.
  • Synthesize evaluation results to formulate diagnoses, goals, prognosis, and plans of care.
  • Establish treatment plans based on evaluations and collaborate with the interdisciplinary team.
  • Assess and modify treatment plans and goals as necessary.
  • Provide education and training to patients and their support systems.
  • Conduct functional training including ADLs/IADLs and therapeutic activities.
  • Oversee support staff including PTA, ATC, and PT aides.
  • Promote JAG Physical Therapy in the community through lectures and presentations.

Requirements

  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state regulated CEU requirements in appropriate state (NY, NJ).
  • Valid CPR License.
  • Experience with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
  • Basic computer skills.
  • Ability to work as a team member and independently with minimal supervision.
  • Maintain a positive, professional demeanor.

Nice-to-haves

  • Experience with EMR software is preferred.
