About The Position

The Primary Care Physician (PCP) at Trinity Health provides comprehensive medical care to participants of the PACE Organization, focusing on the frail and elderly population. This role involves collaborating with an interdisciplinary team to assess patient needs, develop care plans, and manage ongoing medical care. The physician will also participate in on-call rotations and may conduct visits outside of the primary facility, ensuring continuity of care for nursing home residents and other community members.

Requirements

  • Doctorate Degree (MD or DO) with a current and active physician license in the state.
  • CDS Certification and DEA Registration required.
  • Current BLS certification required.
  • Board Certification in Internal Medicine or Family Practice required; Geriatrics preferred.
  • Minimum of one year working with the frail or elderly population required.
  • Valid driver's license and proof of insurance required.
  • Strong communication, problem-solving, and organizational skills.
  • Proficiency with Microsoft Office suite.
  • Ability to adapt to changing priorities and demonstrate critical thinking skills.

Nice To Haves

  • Board Certification in Geriatrics preferred.

Responsibilities

  • Participate as a member of the interdisciplinary team (IDT) in assessments and care plan development.
  • Provide initial and ongoing participant assessments and implement care plans.
  • Refer participants to medical specialists as needed and manage their care in collaboration with the IDT.
  • Oversee and manage participants' use of medical specialists and inpatient care.
  • Review medical outcomes to establish best practices and improve participant health.
  • Advise the Medical Director on accountability of services and resource needs.
  • Assist in developing policies, standards of care, and provide guidance to staff on medical issues.
  • Participate in provider on-call rotation as required.
