About the position

The PrEP Navigator at the Sacramento LGBT Community Center is a full-time role focused on supporting individuals seeking access to health systems, particularly in HIV prevention through Pre-exposure Prophylaxis (PrEP). This position is integral to the sexual health team, providing care coordination and support services to both HIV-negative and HIV-positive individuals, ensuring they receive the necessary assistance in accessing and maintaining PrEP care. The role involves community outreach, education, and collaboration with healthcare providers to enhance access to sexual health resources.

Responsibilities

  • Provide education regarding PrEP/PEP and assess barriers to care.
  • Conduct healthcare provider education on PrEP and biomedical intervention strategies.
  • Offer HIV/STI pre- and post-test counseling and testing.
  • Make referrals to external service providers and assist clients with scheduling visits.
  • Conduct follow-up for adherence counseling and appointment scheduling for PrEP patients.
  • Assess benefits and insurance for clients and aid in accessing payor programs.
  • Collaborate with individuals involved in PrEP care to support programming.
  • Create outreach strategies to engage at-risk populations and link them to care.
  • Participate in outreach activities such as health fairs and Pride events.
  • Provide sexual health education and resources to the LGBT+ community.

Requirements

  • Bachelor's degree in public health, social service, or related field.
  • 1-2 years of experience in case management or 3 years of relevant direct experience.
  • Experience in HIV/STI testing and counseling or prevention services preferred.
  • Excellent communication skills, both verbal and written.
  • Ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office Suite, particularly Outlook, Word, Excel, and PowerPoint.
  • Access to reliable transportation with a valid driver's license.

Nice-to-haves

  • Bilingual in Spanish and English.
  • Certifications in CPR, phlebotomy, and HIV Basic I Counselor preferred.
