About the position

This position serves as Health Share's Tribal Liaison to Native Nations and Indigenous Peoples, including Federally Recognized Tribal Governments, in the three counties served by Health Share of Oregon. This position serves as single point of contact at Health Share of Oregon for Federally Recognized Tribes, the Urban Indian Health Program, and the Oregon Health Authority - Tribal Affairs. This position will coordinate and communicate with network providers, partners, and advisors in the CCO service area to manage, lead and coordinate a range of functions to improve the physical, behavioral and oral health of Native American and Alaska Native CCO members. The position establishes and guides Health Share's strategy and practices to ensure an inclusive and equitable service delivery model, and foster better relationships between Health Share of Oregon, Native Nations, and Indigenous Peoples. The position leads efforts to identify and support Tribal and Native-centered activities to increase accountability to health outcomes through effective collaboration in direct consultation with the Tribal Advisory Council, Tribes and Tribal entities, Health Share collaborative partners, management and staff. In addition to conducting work as necessary within formal government-to-government systems and practices, this position co-creates pathways for community-based organizations serving Native communities also located within Health Share's service region to create programs, make policy recommendations and decisions, and define funding priorities.

Responsibilities

  • Fosters and maintains strong relationships between federally recognized Tribal Governments and the Health Share of Oregon Board of Directors, Community Advisory Council, and Health Share collaborative programs.
  • Develops an outreach and implementation action plan that elevates the visibility, celebrates, and honors the history of Native Americans and Alaska Natives in Oregon.
  • Promotes programs and services designed to improve and achieve health equity parity for Native American and Alaska Native CCO members.
  • Meets regularly with Tribal representative(s) to discuss Tribal/CCO related issues.
  • Serves as main point of contact to the Tribal Advisory Council (TAC) and reports to the TAC at least quarterly.
  • Creates participation pathways for community-based organizations serving Native communities to shape and optimize programs, policy decisions, and funding priorities.
  • Promotes and attends training to expand knowledge of foundational principles to productive collaboration with Tribes.
  • Guides Health Share's strategy and practices to ensure an inclusive and equitable service delivery model.
  • Prepares Health Share of Oregon's leadership to address issues concerning Tribal and Urban Indian Health Programs.
  • Tracks and monitors Tribal governance issues and policy processes that impact Health Share's work.
  • Serves as subject matter expert regarding Tribal history, recognition, consultation processes, customs, and service delivery mechanisms.
  • Serves as lead staff for planning and implementing programs and projects affecting Tribal communities.
  • Strategizes with Health Equity and Engagement Team to plan health equity goals and Tribal Traditional Health Worker priorities.
  • Contributes to the development and implementation of Health Share of Oregon's Community Health Assessment and Community Health Improvement Plan.
  • Establishes and facilitates Health Share's Tribal Health workgroup.
  • Facilitates team-based development of recommendations for resolutions to quality issues in delivery of health care services.
  • Ensures best practices for data use and informatics to identify disparities.
  • Drafts reports and other required documentation to describe impact of Tribal outreach and collaboration efforts.
  • Provides strategic guidance on the consideration and inclusion of tribes in local government planning activities.
  • Serves as a thoughtful and tactical partner/leader with Health Share collaborative leadership.
  • Participates in identifying strategies and initiatives that promote Health Share's integrated efforts within tribal entities.
  • Identifies and establishes potential Tribal relationships and partnerships for Health Share's Community Advisory Council.
  • Provides recommendations and facilitates the resolution of systemic issues between the CCO and Tribal health service providers.
  • Promotes and facilitates Tribal engagement in Health Share's Community Health Assessment and Community Health Improvement Plan.
  • Establishes and maintains professional collaborative relationships with providers and health plan partners.
  • Develops and implements new initiatives, fosters inter-agency collaboration, and creates and monitor contracts with Tribal entities.
  • Drives efforts to identify and support tribal community-centered activities.
  • Demonstrates independent judgment and discretion in frequent contact with public officials and community partners.
  • Collaborates with Tribal culture and prevention programs to inform expansion of Health Share services.
  • Plans, coordinates, manages and monitors progress toward accomplishing objectives through facilitation of cross-sector partner meetings.
  • Researches, creates, and proposes Health Share's training and education action plan components.
  • Attends training courses to expand knowledge of foundational principles for working with Tribes.
  • Develops and conducts training for and delivers technical assistance to providers.
  • Identify pathways to grow Tribal leadership and expand community power and influence.
  • Perform work in alignment with the organization's mission, vision, and values.

Requirements

  • Minimum 5 years' experience in healthcare or social services, including experience working with Native American and Native Alaskan tribal entities.
  • Minimum 1 year experience in strategic planning or course work in strategic planning principles and theories.
  • Experience working with community advisory councils and community health initiatives.
  • Experience in tailored written, verbal, and presentation communication to diverse audiences.
