About the position

Catholic Charities, Diocese of Cleveland is seeking a motivated, mission-driven, and knowledgeable social worker to join the Migration and Refugee Services team. This program is dedicated to resettling approximately 500 refugees annually from various countries, including Bhutan, Burma, Sudan, Somalia, Ethiopia, the Democratic Republic of Congo, Burundi, Eritrea, Iraq, Afghanistan, Syria, Pakistan, and Ukraine. The organization respects the dignity of each person and provides a supportive environment where each individual and culture is celebrated. Building upon the courage and strength of these newcomers, the goal is to empower them to achieve self-sufficiency and independence. As a Refugee Case Manager - Healthcare Navigator, you will be responsible for communicating and coordinating with partnering healthcare providers to ensure that refugees receive the necessary medical attention. This includes scheduling and transporting clients to initial refugee health screenings, as well as scheduling advanced screenings as needed. You will accompany refugees to necessary appointments and teach them how to travel unaccompanied to future appointments. Additionally, you will refer clients for ongoing medical case management as needed and assist school-aged children in receiving all required immunizations for school. You will also help eligible children enroll for WIC benefits and attend WIC appointments. A key aspect of this role involves educating clients on navigating the American healthcare system, including understanding insurance, prescriptions, and follow-up care. You will be responsible for documenting all activities with refugees in individual case files, ensuring that these files comply with agency and federal guidelines. This position requires a compassionate individual who is dedicated to making a positive impact in the lives of refugees and immigrants.

Responsibilities

Requirements

  • 1 - 3 years of experience working with refugees and immigrants.
  • 1 - 3 years of case management experience.

Nice-to-haves

  • Knowledge of Russian, French, or Arabic languages.
