About The Position

The Registered Nurse (RN) is a vital member of the interdisciplinary team at VITAS Healthcare, responsible for addressing the comprehensive needs of hospice patients and their families facing terminal illness. This role involves coordinating care, conducting assessments, and implementing individualized care plans while ensuring compliance with hospice standards and regulations. The RN will also provide education, support, and bereavement assistance, participating actively in team meetings and quality improvement initiatives.

Requirements

• Currently licensed to practice nursing in the state where the VITAS program is located.
• A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Reliable transportation with adequate insurance coverage for driver and passengers.
• Availability of a telephone (cellular or landline).

Responsibilities

• Follow VITAS policies and standards and practice within the scope of the RN license.
• Report any requests to practice outside of the scope of license to the manager/supervisor.
• Supervise nursing tasks delegated to Hospice Aides, LPNs, and Volunteers per the plan of care.
• Complete initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement needs.
• Utilize resources and referrals such as PT, OT, Home Health, and Pharmacy as needed.
• Document care events at the time and location of care unless otherwise directed.
• Provide education and training to patients, families, and caregivers regarding care and pain management.
• Participate in patient care meetings and weekly team meetings.
• Provide effective communication and shift reports through various means.
• Participate in bereavement support and on-call rotation as needed.
• Work with the interdisciplinary group to develop and follow the individualized plan of care.
• Obtain physician orders when necessary and evaluate the outcome of services provided.
• Delegate nursing tasks appropriately and ensure patient safety during delegation.
• Participate in Quality Assessment and Performance Improvement (QAPI) activities and gather clinical data for quality measures.
• Develop and achieve professional growth goals and objectives, attending required training and in-services.
• Represent the nursing profession and VITAS positively in all interactions.
