Resident Care Coordinator

About the position

The Resident Care Coordinator in Memory Care is responsible for overseeing the daily care and well-being of residents within a memory care unit. This role ensures that residents with dementia or other memory-related conditions receive the highest standard of care while fostering a compassionate and supportive environment. The Resident Care Coordinator will work closely with family members, staff, and healthcare professionals to develop, implement, and assess individualized care plans that promote resident independence, dignity, and quality of life.

Responsibilities

• Develop, implement, and monitor individualized care plans tailored to the needs of residents with memory impairments.
• Ensure that residents' physical, emotional, and social needs are being met through consistent monitoring and observation.
• Conduct regular assessments of resident health and well-being, adjusting care plans as necessary.
• Serve as the primary point of contact for residents' families and guardians, providing updates on health, behavior, and daily activities.
• Collaborate with interdisciplinary care teams, including nursing staff, therapists, and doctors, to ensure coordinated care delivery.
• Communicate effectively with residents and families about their care needs and goals, advocating for residents as necessary.
• Provide guidance and support to caregivers, nursing staff, and other team members in the implementation of care plans and memory care best practices.
• Assist in the training of new staff on memory care protocols, resident interactions, and safety measures.
• Offer emotional support to staff who are managing the unique challenges associated with memory care.
• Ensure that all resident care documentation is complete, accurate, and up-to-date in compliance with regulatory requirements and facility policies.
• Monitor and maintain compliance with health, safety, and regulatory standards specific to memory care services.
• Plan and coordinate therapeutic and engaging activities to support cognitive stimulation, socialization, and emotional well-being of residents.
• Encourage and facilitate participation in activities designed to promote memory retention and overall wellness.
• Provide direct care services to residents, including assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding.
• Assist with mobility, transfers, and positioning of residents as needed.
• Ensure that residents are comfortable and safe, responding promptly to any physical or emotional needs.
• Respond to emergency situations and behavioral changes, ensuring appropriate interventions are implemented to maintain resident safety and comfort.
• Assist in de-escalation of challenging behaviors in residents using memory care-specific techniques.
• Attend training courses at community and maintain up to date monthly, quarterly, annual training through designated Learning Management System (LMS).
• Attend all required all staff meetings.
• Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.

Requirements

• LPN required.
• Minimum of 2 years of experience working in memory care, dementia care, or a related healthcare field.
• Experience in a supervisory or leadership role is required.
• In-depth knowledge of dementia, Alzheimer's disease, and other memory-related conditions.
• Strong communication and interpersonal skills, with the ability to effectively interact with residents, families, and team members.
• Ability to manage complex care plans and make quick, informed decisions under pressure.
• Compassionate, patient, and understanding, with a genuine desire to improve the lives of residents.
• Excellent organizational skills and attention to detail.
