About the position

As an RN Case Manager at TrustBridge, Inc., you will play a crucial role in providing compassionate end-of-life care to patients in the West Palm Beach, Loxahatchee, Palm Beach Gardens, and Jupiter areas. This position requires a dedicated nursing professional who is committed to ensuring the comfort and quality of life for patients and their families. You will work in a collaborative, team-driven environment, where effective communication with patients, families, and other caregivers is essential. The ideal candidate will possess a special hospice heart, demonstrating kindness and patience while helping patients celebrate the remainder of their life's journey. In this role, you will be responsible for coordinating care for patients and their families under the direction of the primary physician. This includes conducting thorough assessments of patient and family needs, developing the Interdisciplinary Group (IDG) Care Plan, and ensuring the implementation and coordination of the interventions and services described in the care plan. Your commitment to making the comfort and care of patients and families your highest priority will be paramount. You will consistently provide a high level of personalized, sincere, and engaging service to patients and families, ensuring their complete satisfaction and promptly addressing any concerns that may arise. The hours of operation for this position are from 8:30 AM to 5 PM, with a requirement to work some weekends. Responsibilities

  • Coordinate the care for the patient/family under the direction of the primary physician
  • Conduct assessment of the patient/family needs
  • Develop the IDG (Interdisciplinary Group) Care Plan
  • Ensure the implementation and coordination of the interventions and services described in the care plan
  • Pledge to make the comfort and care of patients and families the highest priority
  • Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times
  • Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.

Requirements

  • RN degree from a college or university accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education
  • Registered Nurse currently licensed in the State of Florida
  • BSN preferred
  • Hospice certification preferred
  • Current Basic Life Support Certification
  • One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
  • Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
  • Basic computer skills and EMR software experience a plus
  • Able to communicate effectively with other Hospice care personnel
  • Able to coordinate with other health care providers in the delivery of patient care
  • Able to work effectively with lay and professional volunteers

Nice-to-haves

  • Bilingual (English/Spanish) preferred
