About the position

As an RN Case Manager at our assisted living facility, you will play a crucial role in delivering high-quality care to our residents. Your primary responsibilities will include coordinating comprehensive care plans tailored to individual patient needs, conducting thorough assessments, and ensuring that all healthcare standards and regulations are met. You will be tasked with managing home visits within your designated service area, which will be divided into North and South Everett, allowing you to provide personalized care to each resident. This position requires a strong commitment to our core values and a dedication to long-term employment, ideally for a minimum of 3-5 years. Responsibilities

Requirements

  • Current and active WA State Licensed Registered Nurse (RN).
  • Valid CPR certification.
  • Valid driver's license and vehicle insurance.
  • Access to a dependable vehicle.

Nice-to-haves

  • Bachelor's degree in nursing.
  • Experience in home health and case management.
