About The Position

The position is responsible for providing quality education, follow-up, and emotional support to patients and their families, managing cases throughout inpatient and outpatient settings. The role involves assessing educational needs, developing curricula, coordinating multispecialty team clinics, and ensuring continuity of care through effective communication and collaboration with healthcare teams and the community.

Requirements

• Bachelor's Degree in Nursing is required.
• 5 years of relevant nursing or CDE clinical experience is required.
• MI Registered Nurse (RN) license is required.
• AHA or ARC Basic Life Support (BLS) certification is required within 90 days.

Nice To Haves

• Master's Degree in Nursing is preferred.
• Prior teaching experience with patients of all ages is preferred.

Responsibilities

• Assess the educational needs of clients and develop appropriate educational curricula or make referrals.
• Case manage the population in both inpatient and outpatient settings, developing clinical paths and measuring outcomes of care.
• Coordinate multispecialty team clinics to enhance patient care.
• Provide direction to projects and programs related to clinical management, evaluating outcomes.
• Document educational processes and patient information according to guidelines.
• Communicate and collaborate with clients, healthcare team members, and the community to ensure continuity of care.
