About The Position

The RN Home Health Manager is responsible for overseeing nursing staff and activities within a community-based home health and hospice service. This role includes managing patient care, staff scheduling, performance evaluation, and ensuring compliance with nursing regulations. The manager will also engage in training staff, coordinating interdisciplinary communication, and managing workflow volumes to enhance service quality.

Requirements

  • Bachelor's degree in nursing required.
  • Graduate from a nationally accredited nursing program preferred.
  • Thorough knowledge of the nursing practice act and standards of nursing care.
  • Two years of nursing clinical experience in caring for patients, residents, or clients preferred.
  • Two years' experience in management duties is preferred.
  • Currently holds an unencumbered Registered Nurse (RN) license with the State Board of Nursing.
  • Basic Life Support (BLS) certification may be required for frequent patient contact.
  • Certified Hospice and Palliative Nurse (CHPN) desirable.

Nice To Haves

  • Certificate for Outcome and Assessment Information Set (OASIS) Specialist-Clinical (COS-C) preferred.

Responsibilities

  • Manage nursing staff and nursing activities for the assigned department.
  • Assign patients/residents to available nursing staff based on condition and skill level.
  • Schedule nursing staff to specific shifts considering past utilization trends.
  • Measure employee performance and assist in training new staff members.
  • Manage workflow volumes, including monitoring and processing work queues.
  • Apply continuous improvement processes and evaluate their impact on quality.
  • Monitor patient/resident care areas for cleanliness and readiness.
  • Provide nursing care to patients/residents as needed.
  • Assist and manage special projects as assigned by leadership.
  • Direct nurse and staff activities for patients/residents receiving home health and/or hospice services.
  • Ensure effectiveness and efficiency of home health and/or hospice services.
  • Delegate responsibilities to clinical and professional staff within home care and/or hospice.
  • Provide direction on nursing practice and home care/hospice regulations to staff.
  • Provide input for planning and budgeting.
  • Coordinate inter-disciplinary communication and collaboration among team members.
