Requirements

• Graduate of an accredited school of nursing.
• Associate degree in nursing.
• Registered as RN with New Hampshire's Board of Nursing.
• One year's recent acute care experience, preferably in med/surg or acute rehab setting.
• Ability to exercise initiative and independent judgment.
• CPR Certification.
• Satisfactory criminal record.
• Satisfactory motor vehicle record.

Nice To Haves

• Bachelor's degree in nursing.
• Community Health/Education experience.
• Certifications: IV, Chemotherapy, Ostomy, as examples.
• Familiarity with OASIS and what drives reimbursement.
• EMR software proficiency.

Responsibilities

• Completes an initial home health assessment of patient and family to determine home care needs.
• Provides a complete physical assessment and history of current and previous illness(es).
• Utilizes the nursing process in planning, providing, and evaluating skilled needs.
• Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Includes the patient and family in the planning and revising process.
• Manages cases of patients to help them heal and regain strength.
