About the position

The Scheduler plays a crucial role in supporting the Administrators or Designated Branch Directors by managing patient visit schedules. This position ensures that direct care team members are scheduled effectively to provide timely patient care, acting as a liaison between various stakeholders including the Clinical Team Manager, direct care team members, and medical providers. The Scheduler's work is vital in delivering quality care to over 65,000 patients nationwide, making a significant impact on their lives and the lives of their families.

Responsibilities

• Manage direct care team member schedules to ensure timely patient care visits.
• Act as an intermediary between the Clinical Team Manager, direct care team members, and medical providers.
• Support Administrators in scheduling patient visits and advancing key workflows throughout the patient journey.
• Respond to and control unexpected situations by evaluating solutions and implementing the best options.
• Seek patient feedback and advocate for patient needs, managing expectations effectively.

Requirements

• High school diploma or GED required; Associate's degree preferred.
• 1 year of practical experience required.
• 1 year of experience in patient scheduling in a home care agency setting preferred.
• Familiarity with national standards of care, state licensure, and federal conditions of participation.
• Ability to convey information clearly and listen effectively.
• Proficiency in enterprise applications (Homecare Homebase, Workday, etc.) and intermediate knowledge of Microsoft Office.
