Requirements

  • Bachelor degree in Human Services field strongly desired.
  • A strong desire and passion to work with people that are unwell, disabled, or living with dementia.
  • Basic understanding of how to support those living with mental health, alcohol/drug addiction, post-traumatic injuries, physical disabilities, and dementia.

Responsibilities

  • Coordinate Social Services in your assigned neighborhood.
  • Promote and enhance relationships between residents, their families and the support systems and resources inside or outside the community.
  • Develop one-to-one relationships with residents and families as needed for support.
  • Refer residents to social, health, mental health, and community agencies and ensure documentation is in the resident's chart regarding the results of referrals.
  • Coordinate family and community resources upon discharge to solve financial needs and promote emotional security.
  • Educate residents, families, and employees concerning resident rights.
  • Utilize the PETI Program and other community resources to ensure eligible residents receive glasses, dentures and other services.
  • Participate in the theft/loss program as well as formal and informal grievance procedures.
  • Complete discharge planning and transfers in accordance with discharge policy.
  • Complete Social Services portion of the resident's MDS within required timeline.
  • Complete a social history of a resident within state required timeline.
  • Document relevant interactions with residents and/or families in resident chart as required by state standards.
  • Involvement in pharma and non-pharma interventions and behavior tracking.
  • Make planned one-to-one visits with residents and document progress in chart.
  • Assist in the development and implementation of relevant policies and procedures.
  • Follow through with PaSSRR recommendations.
  • Assist residents to find meaningful work and a sense of purpose in the community.
  • Develop the social care component of the Care Plan for each resident.
  • Ensure resident and staff participation in their care plan.
  • Collaborate with Residents and staff to develop non-pharmalogical interventions.
  • Support residents and families in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
  • Provide resources inside and outside of the community; working knowledge of advanced directives and palliative care.
  • Collaborate with nursing to ensure the proper care of residents.
  • Act as a liaison with social, health and community agencies.
  • Investigate all reports of potential abuse and neglect.
  • Be familiar with disaster planning policies and procedure.
  • Comply with HIPAA and Confidentiality policy at all times.
  • Promptly report any occurrences or potential abuse immediately.
  • Approach all encounters with residents, guests, and employees in a friendly, service-oriented manner.
  • Maintain regular attendance as required.
  • Maintain high standards of personal appearance and grooming, which include wearing the proper uniform and nametag when working.
  • Comply at all times with the standards, policies, and regulations to encourage productive, safe, and efficient community operations.
  • Attend in-service education, care conferences, and meetings as required.
  • Other duties as assigned.
