The Social Work Care Manager will work closely with interdisciplinary teams, including physicians, nurses, and care managers, to provide patient-centered care coordination throughout the continuum of care. This role is essential in facilitating the development of discharge plans for high-risk patient populations, ensuring that all aspects of a patient's care are considered and addressed. In this position, the Social Work Care Manager will receive referrals from various team members for patients who require assistance with eligibility determination for social programs and funding sources. They will also provide crisis intervention for patients and families facing psychosocial challenges. The Social Work Care Manager is responsible for conducting patient evaluations post-hospitalization, developing transition of care plans, and implementing these plans prior to patient discharge. This role emphasizes the importance of optimal patient flow and throughput, aiming to enhance continuity of care, improve patient satisfaction, and prevent readmissions. The Social Work Care Manager will actively participate in daily multidisciplinary rounds, ensuring effective communication and collaboration with the healthcare team. They will also provide education on resource utilization, medical necessity, and compliance with regulations related to discharge planning. Knowledge of post-hospital care services, including home health, hospice, and rehabilitation services, is vital for this role. The Social Work Care Manager is expected to adhere to departmental goals and policies while maintaining a commitment to outstanding customer service and respectful relationships with all stakeholders.

Responsibilities

  • Intervene with patients who have complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources.
  • Provide crisis intervention to patients and families with psychosocial needs.
  • Coordinate and facilitate the development of discharge plans for high-risk patient populations.
  • Receive referrals from interdisciplinary team members for at-risk populations.
  • Ensure patient-centered care coordination through the continuum of care.
  • Monitor resources and escalate clinical care as needed for efficient and cost-effective care.
  • Conduct patient evaluations of post-hospital needs and develop transition of care plans.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.

Requirements

  • Master's degree in Social Work or related field.
  • Minimum of 3 years of experience in social work or case management.
  • Experience in discharge planning and care coordination.
  • Knowledge of community resources and post-hospital care services.

Nice-to-haves

  • Experience with crisis intervention and mental health services.
  • Familiarity with advanced directives and ethical dilemmas in patient care.
