About the position

The Case Manager plays a crucial role in advocating for and assisting clients in accessing essential medical, social, educational, and other services to meet their identified needs and achieve specific goals. This position is centered around providing ongoing assessment and support to engage potential clients, particularly during critical engagement points. The Case Manager is responsible for offering referrals and information to enhance education for clients and their families regarding available resources, thereby increasing support networks and coordinating care among various service providers. These providers may include Drug Court, Mental Health/Substance Abuse Treatment Teams, the Department of Community Based Services, Juvenile Justice, schools, and healthcare providers. As a member of a specialized team, the Case Manager addresses population-specific areas such as adolescent and adult substance use, seriously mentally ill adults, or youth with severe emotional disabilities. Regular direct and indirect contact with clients and significant individuals in their lives is essential to this role, ensuring that the Case Manager can effectively support and advocate for their clients' needs. The position requires a proactive approach to case management, including the development of Person-Centered Recovery plans of care, ongoing monitoring of service delivery, and advocacy on behalf of clients and their families. Collaboration with team members and community partners is vital to help clients identify and attain their goals, while also serving as a community liaison to foster relationships that support clients in building their networks of family, friends, and community resources. Documentation is a critical component of this role, as the Case Manager must maintain accurate and thorough records on all assigned cases, ensuring compliance with medical necessity and joint commission standards. Timely documentation is necessary to assist clients in evaluating their progress and to inform supervisors and team members of any issues or concerns related to individual clients.

Responsibilities

  • Advocate for clients and assist them in gaining access to needed services.

  • Collaborate and coordinate resources for individual clients.

  • Attend community meetings to provide education and resources.
  • Document to meet medical necessity and joint commission standards.
  • Maintain accurate records on all assigned cases.

Requirements

  • Bachelor's degree in psychology, social work, or a related behavioral science field.
  • One year of full-time employment experience working directly with adults/children in a human service setting post-degree (for SMI Adults and SED Child case managers).
  • One year of full-time employment experience working directly with individuals in a human service setting post-degree (for Substance Use case managers).
  • Proficiency in American Sign Language for Deaf and Hard of Hearing case managers, with an Advanced or above rating on the SLPI.
  • Completion of a Targeted Case Management certification program within six months of employment.
  • Valid driver's license, reliable transportation, and proof of current auto insurance.

Nice-to-haves
