About the position

The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient's plan of care from admission to post-discharge. The Social Worker utilizes critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, effective and efficient transition of care across the continuum.

Responsibilities

• Assess patients' needs and develop a comprehensive plan of care.
• Coordinate and implement discharge plans for patients.
• Monitor and evaluate the effectiveness of the care plan.
• Utilize critical thinking skills in case management.
• Ensure a seamless transition of care across the continuum.

Requirements

• Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia.
• Master's degree in social work.
• Three years' experience in acute care or other related healthcare field is preferred.
• Effective oral and written communication and interpersonal skills.
• Knowledge of government and third-party reimbursement systems and contracts.
• Strong organizational and problem-solving skills.
• Ability to maintain confidentiality.
• Ability to adapt to multiple and changing priorities.

Nice-to-haves

• Case Management Certification preferred.
• Previous experience in case management and/or discharge planning is strongly preferred.
