About the position

BJC HealthCare System is hiring a Full-Time Social Worker (MSW) Intake Coordinator for our Medical Stabilization Services located at the Parkland Health Center (Farmington, MO). As part of BJC HealthCare, Parkland Health Center offers a unique combination of world-class medicine along with the deep local roots and rich history of its three predecessors - Bonne Terre Hospital (opened in 1911), Mineral Area Regional Medical Center (1952), and Farmington Community Hospital (1969). Parkland Health Center has two locations in St. Francois County, Missouri - a full-service acute care hospital in Farmington as well as a rural emergency hospital in Bonne Terre. Parkland Health Center provides quality primary and secondary level inpatient and outpatient services to the people of St. Francois County and surrounding areas in southeastern Missouri.

Responsibilities

  • Coordinate intake and admitting procedures for chemical dependency services
  • Ensure referral source satisfaction
  • Provide evaluation documentation utilizing available data
  • Link patients to treatment and ensure smooth transition to the next level of care
  • Provide support for supplementary social work requirements and/or tasks related to discharge planning
  • Provide direct clinical services to patients and families
  • Plan and implement social work services on assigned clinical service or unit in collaboration with the healthcare team
  • Collaborate with community human service agencies to meet the needs of patients, families, and the hospital

Requirements

  • Master's Degree of Social Work (MSW) - required

Nice-to-haves

  • Social Work experience in Chemical Dependency and/or Emergency/Inpatient Medical Care (preferred)
  • 2-5 years of experience (preferred)
