About the position

The Care Coordinator is responsible for ensuring smooth patient flow within the healthcare facility by managing appointments, facilitating communication between patients and healthcare providers, and maintaining compliance with healthcare regulations. This role emphasizes customer service, data management, and collaboration with interdisciplinary teams to enhance patient care and outcomes.

Responsibilities

• Schedule and reschedule patient appointments to ensure smooth patient flow.
• Answer phone calls promptly and multitask effectively.
• Evaluate care outcomes with the interdisciplinary team and implement recommendations.
• Engage and attract patients while providing excellent customer service.
• Maintain a patient care database and verify findings and reports.
• Ensure compliance with healthcare regulations and standards.
• Orient and educate patients and their families about care plans and health conditions.
• Develop interdisciplinary care plans and coordinate information with other care providers.
• Encourage open dialogue and active participation among team members.

Requirements

• 3-5 years of experience in clinical or community resource settings.
• Experience in care coordination and/or case management is desirable.
• Strong leadership, communication, education, and counseling skills.
• Proficiency in communication technologies (email, cell phone, etc.).
• Highly organized with the ability to keep accurate notes and records.
• Experience with health IT systems and reports is desirable.
• Local knowledge about community health care and social welfare resources is desirable.
• Fluency in Spanish and English is required.
• Advanced knowledge of Excel and EHR is preferred.
