About The Position

The Special Needs Case Manager at Parenting Network Inc. is a part-time role focused on providing case management and support services to families with children diagnosed with special needs. The position requires the individual to have personal experience as a parent, grandparent, or sibling of a child with special needs, enabling them to offer empathetic and informed support. The case manager will work closely with families to identify their needs, provide resources, and empower them with skills to advocate for their children effectively.

Requirements

• Bilingual in Spanish preferred.
• Bachelor's degree in social work or related field preferred.
• Minimum of 2 years of experience in case management preferred.
• Must be a parent, grandparent, sibling, or relative of a child diagnosed with a special need.
• Valid California Driver's License required.
• Ability to pass a criminal background check required.

Responsibilities

• Provide feeling-focused parent-to-parent support.
• Complete necessary documentation for each special needs individual in the client's care.
• Conduct home and office visits to each family being case managed.
• Offer transportation services as needed.
• Complete case management tools and documentation.
• Deliver food, clothes, and other basic needs to families.
• Keep case notes and documentation current.
• Provide education and support on IEP/504 plans as applicable.
• Educate parents on how to be successful advocates for their child.
• Address client's needs with referrals and assist with forms and applications for services (SSI, CCS).
• Schedule client appointments with various social service agencies.
• Perform additional duties as assigned.
