About the position

The Speech Language Pathologist (SLP) position is a full-time role located in Jacksonville, Florida, specifically within a home health care setting. This position offers a $10,000 sign-on bonus contingent upon a two-year commitment. The SLP will work on a per-visit model, providing flexibility and the opportunity to engage with patients in their own homes. The role emphasizes collaboration within a friendly and supportive team environment, where the SLP will work alongside various healthcare professionals to deliver superior patient care. In this position, the SLP will have the chance to collaborate with specialty teams that focus on diverse areas such as neurological rehabilitation, geriatrics, orthopedics, pediatrics, sports therapy, women's health, vestibular/balance issues, oncology, and low vision. This multidisciplinary approach ensures comprehensive care for patients with a wide range of needs. The SLP will be responsible for conducting new patient evaluations, establishing customized treatment plans, and setting patient goals, all while participating in the implementation of patient care programs. The SLP will apply various therapeutic techniques to rehabilitate patients suffering from speech and language disorders, hearing disorders, and swallowing disorders. Delivering high-quality patient care is paramount, and the SLP will strive to provide a positive patient experience through engagement and progressive treatment methods. Effective communication, both verbal and written, is essential in this role, as is the promotion of evidence-based patient care practices.

Responsibilities

  • Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs
  • Apply a variety of therapeutic techniques for the rehabilitation of patients with speech and language disorders, hearing disorders, and swallowing disorders
  • Deliver high quality patient care for multiple rehabilitation diagnoses
  • Provide a positive patient experience through patient engagement and progressive treatment
  • Effectively communicate both verbally and written
  • Promote evidence based patient care

Requirements

  • Current SLP license in state of Florida
  • Current hands-on CPR/BLS Certification
  • Knowledge of federal and state professional requirements
