About the position

The Support Coordinator position at MOCEANS Center for Independent Living is a full-time role that operates on a 4-day work week, focusing on providing essential services to individuals with intellectual and developmental disabilities in Monmouth and Ocean Counties. The coordinator will develop a comprehensive understanding of the services funded by the Division of Developmental Disabilities (DDD) and ensure that these services are utilized effectively and appropriately. This role involves establishing contact with individuals, their families, and support systems to facilitate a person-centered approach to service delivery. Key responsibilities include interviewing assigned individuals and their family members or guardians, scheduling and facilitating planning team meetings, and completing individual service plans (ISPs). The coordinator will review and compile assessments and evaluations, identify necessary supports, and facilitate the completion of discovery tools. They will also be responsible for writing and distributing the ISP to identified service providers, monitoring service delivery, and ensuring that the rights and needs of individuals are respected and met. The position requires regular contact with individuals through phone calls, quarterly face-to-face meetings, and annual home visits. The coordinator will maintain confidential case records and other supporting documents, ensuring that individuals are free from abuse and neglect, and will report any suspected cases as necessary. Participation in staff and interdisciplinary team meetings is also expected to provide feedback and support to both consumers and staff. The role may require on-call coverage as assigned, emphasizing the need for flexibility and responsiveness to the needs of individuals served.

Responsibilities

  • Develop a thorough understanding of the services funded by the DDD and ensure these services are utilized appropriately.
  • Establish contact with the individual and family and support system as assigned.
  • Interview the assigned individual and any appropriate family members or guardians.
  • Schedule and facilitate planning team meetings.
  • Complete person-centered planning tool and individual service plans.
  • Review and compile assessments/evaluations, identify supports and facilitate completion of discovery tools.
  • Write and distribute the ISP to the identified service providers and regularly review the ISP.
  • Monitor and follow up to ensure delivery of quality services in full consideration of the individual's rights and needs.
  • Conduct monthly contact by phone or in person, quarterly face-to-face meetings and annual home visits with each individual as assigned.
  • Organize and maintain confidential case records and other supporting documents.
  • Ensure individuals are free from abuse and neglect, report suspected abuse or neglect and provide follow-up as necessary.
  • Participate in staff meetings and interdisciplinary team meetings to provide feedback and support to consumers and staff.
  • Develop and maintain professional relationships with appropriate services and agencies.
  • Provide on-call coverage as assigned.

Requirements

  • Must have a Bachelor's Degree; a degree in Psychology, counseling, social work or human services is preferred.
  • Must have 1 year documented experience either paid or volunteer working with adults with intellectual or developmental disabilities.
  • Advanced computer skills including typing, Microsoft Office Suite and/or Google Apps.
  • Excellent verbal, written, and organizational skills.
  • Knowledge of the resources and services available to people with disabilities in Ocean and Monmouth counties in New Jersey.
  • Must be able to meet with individuals in their homes and in the community.
  • Must be able to drive throughout Monmouth and Ocean counties or have reliable transportation.
