About the position

The Team Leader for the Comprehensive Class Member Transition Program (CCMTP) is a managerial role responsible for overseeing the Transition Team, ensuring quality service delivery to Class Members transitioning from institutional settings to community living. This position involves direct supervision, training, and support for team members, while collaborating with interdisciplinary teams to advocate for Class Members and link them to necessary community services. The Team Leader also plays a crucial role in implementing best practices and maintaining compliance with program standards.

Responsibilities

  • Supervise, coach, and ensure productivity of the Transition Team, including Transition Coordinators and specialty support roles.
  • Lead and oversee community support activities, transition linkage, and case management services provided by the treatment team.
  • Assist in community integration, skill building, transition preparedness, and overall recovery support.
  • Establish and implement best practice protocols in the CCMTP.
  • Develop and update necessary templates/forms in collaboration with the CCMTP Program Director.
  • Identify areas for improvement and develop action plans in collaboration with leadership.
  • Provide direct hands-on training and recommendations for additional staff training.
  • Attend leadership meetings and trainings to enhance leadership skills.
  • Provide individual and group supervision to team members for clinical case management and service delivery.
  • Facilitate team meetings to ensure quality service delivery and identify areas for improvement.
  • Create and manage the weekly staffing schedule to support transition work.
  • Support staff with time management to meet documentation and productivity requirements.
  • Ensure compliance with Thresholds procedures regarding members' transition funds.
  • Complete required training and maintain certifications for assessments and documentation.
  • Participate in the development of Class Member Comprehensive Service Plans.
  • Maintain confidentiality of personal health information according to HIPAA and Thresholds standards.
  • Monitor and coach staff on meeting productivity measures and documentation timelines.
  • Conduct annual evaluations for team staff and assist with onboarding new employees.

Requirements

  • Bachelor's degree in social work, counseling, psychology, or a closely related field (Master's preferred).
  • Two to five years of clinical experience preferred.
  • Leadership experience preferred.
  • Computer proficiency, including electronic medical records and relevant software.
  • Strong interpersonal skills and knowledge of group dynamics.

  • Current and valid Driver's License
  • Access to a personal vehicle for program use.
