About the position

The Team Manager is responsible for leading the Patient Care Team in a nursing home setting, ensuring the delivery of high-quality patient care. This role involves supervising and coordinating the interdisciplinary team, advocating for patients, and maintaining comprehensive patient records throughout their care journey. The Team Manager also oversees staff management, quality improvement initiatives, and customer service, ensuring compliance with regulatory standards and effective resource utilization.

Responsibilities

  • Supervise, evaluate, and coordinate the interdisciplinary team members.
  • Ensure continuity of care from admission to discharge or transfer to bereavement.
  • Serve as a patient advocate and coordinator for social service and healthcare providers.
  • Maintain patient records from intake through discharge or transfer to bereavement.
  • Direct all patient care services for the team, including volunteer and bereavement services.
  • Review pending patient admissions and ensure eligibility and appropriateness criteria are met.
  • Collaborate with physicians regarding palliative care measures for pain and symptom management.
  • Monitor team services provided to ensure they meet patient and family needs.
  • Manage the team schedule and coordinate services 24/7.
  • Facilitate team meetings to update and revise care plans based on patient needs.
  • Participate in on-call rotation for patient care support.
  • Interview, select, train, supervise, and evaluate team staff.
  • Ensure staff competence through field visits and documentation evaluations.
  • Oversee staff schedules and territory assignments for equitable workload distribution.
  • Review and approve payroll for team members.
  • Monitor clinical records to ensure compliance with VITAS policy and regulatory standards.
  • Develop and implement performance improvement activities.
  • Address customer service issues and grievances promptly and satisfactorily.
  • Analyze customer service issues to identify causes and improve performance.
  • Regularly visit LTC and Contract Bed facilities for care plan integration and customer satisfaction.
  • Participate in professional, voluntary, or community service organizations.
  • Verify and approve admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitor resource utilization to ensure cost-effective service delivery.
  • Attend inservices, educational seminars, and workshops for professional development.

Requirements

  • Current and valid RN License to practice in the state where the VITAS program is located.
  • Bachelor's degree preferred.
  • Two years of successful supervisory experience or equivalent in a healthcare organization.
  • Knowledge of primary medical and nursing care principles, particularly in oncologic nursing and palliative care.
  • Understanding of interpersonal dynamics within an interdisciplinary healthcare team.
  • Familiarity with the VITAS information system (Vx) and management reports.
